What is breast augmentation?

Breast augmentation, also called breast enlargement, is a plastic surgery procedure that fills out the breasts with silicone or saline breast implants or fat transfer. It is one of the most popular procedures Dr. Newman performs.

How much does breast augmentation cost in Westchester, Long Island & NYC?

For breast augmentation performed by Dr. Newman, the typical fees start at $12,500. These costs include everything necessary for surgery, such as Dr. Newman’s surgical fees, anesthesiologist fees, and all facility fees. Depending on the implant you choose, there may be an additional fee for the cost of the implants themselves. The price also includes all visits before and after surgery, surgical photography (before and after photos), and the surgical bra you will need. Am I a candidate for breast augmentation?

Breast Enhancement and Pregnancy

While the desire to become pregnant down the road does not rule you out as a candidate for breast implant surgery, it is a factor that should be considered when deciding whether to undergo the procedure. Future pregnancies can potentially impact your results, diminishing the effectiveness of your procedure.

In general, if you are planning to have children in the very short term, it is a good idea to wait until you are done with childbirth before undergoing breast augmentation. To ensure that you fully understand the impact of future pregnancies on your results, you should discuss these plans with Dr. Newman at your initial consultation.

Breast Implants and Teenagers

The FDA has approved saline breast implants for use in women age 18 and over and silicone breast implants for use in women who are at least 22. However, meeting these minimum age requirements does not necessarily ensure that you are a suitable candidate for breast augmentation. There are multiple factors that must be evaluated when breast augmentation is being performed on a teenager or young adult.

What can I expect at a breast augmentation consultation?

During your initial consultation, you will meet with Dr. Newman for 30 to 45 minutes. He will review your medical history, perform a physical exam, and ask you about your goals and expectations for the procedure. This is a time for you to take your time to:

Understand your options: Dr. Newman will explain the surgery and the many options available to you, including breast implants. He will use VECTRA® 3D imaging to help you visualize your possible breast augmentation results with various implant types and sizes. You will learn about the disadvantages or advantages of certain techniques and implant types. Together, you will be able to determine which options best suit your body, goals, and lifestyle.

Ask questions: You will find the consultation is very complete and that many of your questions will be answered without even having to ask. Still, it is important to get all your questions answered. Patients who do not write down a list of questions will usually forget them, so feel free to bring a list with you.

Review before-and-after photos: You will have time to look through photos of Dr. Newman’s patients who have similar characteristics to you so you can see how their particular cases were approached. You can feel free to bring in photos from the web or magazines to discuss as well.

Learn about your next steps: After meeting with Dr. Newman, you will meet with his patient care coordinator Kerri. Where is the incision for breast implants?

Many breast augmentation patients worry about the visibility of the scars after healing is complete. Dr. Newman usually uses very small incisions, only about an inch long, which are easily hidden. He usually places the incision in one of the following locations:

  • In the fold beneath the breast (inframammary incision)
  • Under the arm (transaxillary incision)
  • Along the edge of the areola—the darker skin around the nipple (periareolar incision)

Some procedures will require more incisions, and if this is the case with your procedure, Dr. Newman will discuss incision placement with you during your breast augmentation consultation.

Where is a breast implant placed?

Another important consideration for breast augmentation is the placement of the implant. Depending on the shape and size of your breast, Dr. Newman will determine whether the implant should be placed beneath the chest muscle (submuscular placement) or on top of the chest muscle and beneath the breast’s glandular tissue (subglandular placement).

  • Submuscular placement of the implant is the most common. Placing the implant beneath the muscle reduces the likelihood that the implant itself will be visible. It also reduces the occurrence of rippling, which occurs more commonly in saline implants, because the implant is covered by the muscle tissue.
  • Subglandular placement is sometimes used when the breasts are small and there are insufficient tissue and skin available to cover the implant if it is placed beneath the muscle. Rippling is more likely with this placement. It is also more likely that the implant will be visible beneath the skin.

In some cases, Dr. Newman uses a supportive mesh called GalaFLEX as an internal “bra” to support the implants and maintain upper breast fullness. The meshing is a very thin material inserted into the breast pocket to support tissues and encourage the production of collagen and elastin. Within 12 to 18 months, the meshing dissolves, much like absorbable sutures.

What kind of anesthesia is used for breast augmentation?

Breast augmentation can be performed by Dr. Newman using general anesthesia, IV sedation (“twilight”), or local anesthesia. The choice of anesthesia is based mostly on the patient’s preference, as well as Dr. Newman’s decision.

At Dr. Newman’s private surgical facility, anesthesia other than local anesthesia is administered by board-certified anesthesiologists with extensive experience specifically in anesthesia of the ambulatory surgery patient.

Does breast augmentation require general anesthesia?

Dr. Newman believes breast augmentation surgery should only be performed using general anesthesia for several reasons. Most importantly, he believes you will get a better result using general anesthesia for your procedure. Other points are:

  • Contrary to popular opinion, general anesthesia administered by an experienced board-certified anesthesiologist is very controlled and safe. Most anesthesiologists would agree that general anesthesia, administered in the proper environment, is actually safer and better controlled than IV sedation.
  • IV sedation will make you sleepy but won’t stop you from feeling pain and discomfort the way general anesthesia can. Therefore, a local anesthetic must be given as well. It is difficult to anesthetize the entire breast using local anesthesia, especially when the procedure involves implants placed under the pectoralis muscle.
  • Typically, at least 50cc of local anesthetic is needed to anesthetize each side adequately. Dr. Newman believes that the local anesthesia alone changes your breast size and shape significantly. Simply administering the local anesthetic additionally causes immediate swelling and bruising that also changes the appearance of your breast.
  • Even though local anesthetic is used when breast augmentation is performed using IV sedation, most patients will shift their weight every few minutes. A mere half-inch move of one shoulder changes the position of one breast compared to the other by a full inch.

Dr. Newman’s Opinion: About Anesthesia

“My ability to give you the best result possible depends critically on my ability to judge your appearance in the operating room. It’s my opinion that using local anesthesia for cosmetic breast procedures changes your appearance so significantly that it affects my ability to give you a reliable result. There is no question that avoiding an anesthesiologist for your breast augmentation surgery will lower the cost of the surgery. I feel strongly, especially for breast augmentation, that the benefits of having an anesthesiologist present for your surgery are definitely worth the cost.

“From the perspective of safety, I can tell you that I have operated on family, friends, other doctors, and their families in this facility using this same anesthesiology staff. In fact, I had surgery in this facility myself.”

How do I prepare for breast augmentation?

Once you have scheduled a date for your breast augmentation with Dr. Newman, you may still have questions about the details of what else to do. We encourage you to ask as many questions as you need so that you are as comfortable as possible on the day of your breast augmentation. Below are some of the more practical answers to a few of the questions you may have.

Christine Jewell, R.N., the Director of Surgical Nursing, will contact you the day before surgery with an exact time to arrive at the center. If our office is arranging transportation for you, then she will give you all the necessary information.

What should I wear to the surgery center or hospital for breast augmentation surgery?

  • Wear sweatpants or loose-fitting jeans.
  • Wear a button-down or zip-up top; avoid tops that pull over your head.
  • If possible, wear underwear that is 100% cotton.
  • Bring socks and comfortable shoes that slip on.
  • A surgical bra will be provided for you and is usually already on when you wake up from surgery.

Can you have breast augmentation while on your period?

If you have your period on the day of surgery, please bring several tampons or pads.

Can I take my normal medication on the day of surgery?

Dr. Newman will give you prescriptions during your pre-operative visit. Please get these filled and bring all medications with you on the day of surgery. If you take medication other than what Dr. Newman has prescribed, please bring a list of your medications with you on the morning of surgery. This includes vitamins, over-the-counter medications, and supplements. We need to know everything you take. The anesthesiologist will ask you for this information when they call you a day or two before your surgery.

Should I wear my glasses/contact lenses on the day of surgery?

If you wear contact lenses, please do not wear them on the day of surgery. Please wear your glasses and bring a case with you.

Forms

The consent forms required for surgery will be given to you during your pre-operative visit. Please read them and bring them back with you on the day of surgery. Dr. Newman will review them and answer any questions you have before you sign them.

How long does it take to recover from a breast augmentation?

Dr. Newman’s breast augmentation patients are typically back to work and a relatively normal routine within 3 days.

Soreness

While experiences vary among individuals, most soreness will disappear within the following time frames:

  • 2 to 3 months (implants above the chest muscles)
  • 6 months to a year (implants placed beneath the chest muscle)

Bras

Since the breasts will be heavier after breast augmentation, wearing a comfortable bra that fits well can help support the breasts and contribute to faster, more comfortable healing.

Breast Augmentation FAQ

Are breast implants safe?

Dr. Newman uses only FDA-approved breast implants that have undergone extensive safety testing. Saline implants are filled with a harmless, sterile saline solution, so you will know right away if there is a leak or rupture. Today’s silicone implants contain a thick gel that holds together if the implant has a leak.

Will my breast implants need to be replaced eventually?

Breast implants are not designed to be lifetime devices; most last up to 15 years. Dr. Newman can perform revision surgery if you have concerns, experience complications, or want to “upgrade” your implants for a different type or size.

Is breast augmentation recovery painful?

Dr. Newman prescribes pain medication that’s enough to keep most patients comfortable, although pain tolerances vary. Most of his breast augmentation patients are back to work, school, and normal daily activities within 3 days without significant discomfort.

When can I start exercising again?

Patients can start gentle walking the day of surgery and gradually work up to more vigorous walking as is comfortable. Dr. Newman typically clears patients to resume aerobic exercise approximately 10 days after breast augmentation.

How do you sleep after breast augmentation surgery?

You should sleep with your upper body elevated for the first few days after surgery to minimize swelling and discomfort. Many patients prop themselves up with pillows or sleep in a recliner. After that, it will be best to sleep on your back or side and avoid sleeping on your stomach until cleared by Dr. Newman.

What’s the difference between a breast augmentation and breast implants?
